电流感觉阈值在诊断神经源性敏感性皮肤中的意义

[摘要]目的：探讨电流感觉阈值(CPT)在诊断神经源性敏感性皮肤中的意义。方法：初步筛选60名自我感知为敏感性皮肤的受试者，再分别采用乳酸试验、辣椒素试验半主观评判阳性和阴性结果进行分组，乳酸试验或辣椒素试验阳性为敏感性皮肤，二者均为阴性为正常对照组。其中辣椒素试验阳性提示神经源性敏感性皮肤，并采用CPT检测方法评估各组神经敏感性。

结果：辣椒素试验阳性组和正常对照组CPT值在5Hz频率下分别为(6.67±6.37)和(11.90±8.37)，Hz频率下分别为(12.67±7.61)和(26.33±13.75)，2个频率两组间差异均有统计学意义(P0.05)。结论：辣椒素试验可以反映皮肤神经敏感性的改变，

CPT测试可以客观地反映皮肤神经敏感性改变的程度。故辣椒素试验结合CPT测试可以评估皮肤神经敏感性。敏感性皮肤与无髓鞘C和有髓鞘Aδ两种神经纤维有关。

Objective：Toinvestigatetheclinicalvalueofcurrentperceptionthreshold(CPT)inthediagnosisofneuralsensitiveskin.Methods：Accordingtotheresponsestolacticacidandcapsaicintest，60subjectswithself-reportedsensitiveskinweredividedintotwogroups.Capsaicinorlacticacidtestpositivesubjectsweresensitiveskingroupandbothtestnegativesubjectsservedasnormalcontrols.Capsaicintestpositivewasclassifiedasneuralsensitiveskin.TheskinnervesensitivityineachgroupwasassessedwithCPT.Results：TheCPTateither5HzorHzwassignificantlylowerincapsaicintestpositivesubjectsthaninnormalcontrolsfat5Hz6.67±6.37forcapsaicintestpositiveand11.90±8.37fornormal；atHz12.67±7.61forcapsaicintestpositiveand26.33±13.75fornormal；P0.05forbothfrequencies).Conclusion：Thechangeofskinnervesensitivitycanbereflectedbythecapsaicintest，andtheextentofobjectivechangesinthenervesensitivitycanbedetectedbytheCPTtest.ThecapsaicintesttogetherwiththeCPTcanbeusedtoevaluatethecutaneousnervesensitivity.SensitiveskinisassociatedwithbothunmyelinatedCfibersandmyelinatedAδfibers.
